Before we can begin your representation, we are required to have a signed retainer agreement on file.
- Please fill out the following form as completely and accurately as possible.
- Once received, we will draft your retainer agreement based upon the information you provide, and we will forward your retainer to you by email.
- Once you receive your retainer agreement via email, please print it, initial and sign it, and mail the original copy to us at our mailing address on the letterhead on the retainer.
- Please also print, read, fill out and sign the recurring payment authorization form.
